The following questions allow us to better understand the participants' needs and to provide a safe environment for all our Promise Land participants. Any information shared will be communicated only to those who are directly caring for the individual and only on a need-to-know basis.

You will NOT be able to save and later complete registration so please make sure you have all the following information:

Toileting Assistance
Walking/Standing Assistance
Behaviors/Triggers
Signed Physician Action Plan for those with Asthma, Seizures, or EpiPens (Effective Date of Action Plan cannot be older than one year).

PLEASE NOTE:
In order to ensure our team is adequately prepared to care for your Promise Land participant, the registration form, and signed Consent, Waiver and Acknowledgment must be submitted, reviewed and approved prior to the participant’s attendance at Promise Land.

You will not be able to drop off your participant in Promise Land until one of our staff members has reached out to you regarding their first day of attendance.
