The time required to complete this registration for each person may take 15-30 minutes depending on the participant's medical history. Please note, you will not be able to save and come back to complete registration. A payment MUST be made to complete registration. 

You will need the following information readily available to complete this registration for each student attending.

• Child details
• Emergency Contact Info
• Medical History
• Allergies
• Immunization Information
• List of Medications (RX or OTC)
• Insurance Information (if applicable)
• Picture or Scanned Image of the Front and Back of the Insurance Card
• Physician Provided Care Plans for those with Diabetes, Asthma, Seizures, or EpiPens. 
• Complete payment. Payment options can be full or you set up a payment plan.
